How accurate information reaches leadership with less meaning than it started.
THE DEFINITION
Truth Drift
William Gulley, The Data Trap (2026)
is the loss of meaning as information travels upward.

WHAT IT IS
The data is accurate. The story behind it does not survive the climb.
Truth Drift™ develops because information has to travel through the organization before it reaches the people making the decisions. At each level, the information is summarized based on what they believe the next level needs to know.
Nobody hides anything. Nobody manipulates the numbers. Each person accurately passes along what they believe matters and still leaves leadership with less meaning than the organization started with.
By the time a performance result reaches a dashboard in a leadership meeting, the story that produced it is considerably thinner than the one the people closest to the work would tell.
ILLUSTRATIVE EXAMPLE · WHAT HAPPENED · WHAT WAS SAID ABOUT IT
A department finishes the year with strong productivity and lower overtime.
WHERE IT STARTED
The floor
"We hit the number. Here's what it took, people picked up additional shifts, education was deferred, managers covered weekends, vacancies went unfilled while the team absorbed the work. We need you to understand this before you set next year's targets."
FULL CONTEXT
LEVEL 1
Manager's summary to director
"Productivity was strong despite some staffing challenges. The team really came through."
CONTEXT BEGINS TO NARROW
LEVEL 2
Director's report to executive team
"Productivity exceeded target."
MOST CONTEXT REMOVED
DASHBOARD
What leadership sees
✓ Productivity: Target Met
RESULT REMAINS
WHAT DISAPPEARED ON THE WAY UP
Additional shifts
Deferred education
Managers covering weekends
Unfilled vacancies
Work absorbed by teams
Whether this is sustainable
What next year's target should be
The manager accurately summarized what they believed the director needed to know. The director accurately reported what they believed the executive team needed to know. The dashboard accurately recorded the outcome.
Nobody hid anything. The meaning disappeared anyway.
That is Truth Drift™.
WHY IT MATTERS
The version that reaches leadership becomes the basis for the next decision
Leadership sees the performance and sets next year's expectations from it. What people were able to absorb becomes the new expectation for what the operation can produce. The conditions that made exceptional performance necessary, and the human cost of producing it, disappear from the data entirely.
Truth Drift™ helps explain why a performance problem can return despite capable people, focused attention, and repeated attempts to address it.
The metric is accurate. The interpretation of what produced it is incomplete. The initiative launched in response addresses the incomplete interpretation. The conditions creating the actual problem remain untouched.
William Gulley, The Data Trap (2026)
"Nobody has to manipulate the information. Nobody has to hide a problem. Meaning can disappear a little at a time as a complicated story gets turned into something that can fit into a report, dashboard, or leadership discussion. The final version can be accurate and still be missing what leadership needed to know."
MOVING UPSTREAM
Four questions I use when the number and the work don't match
When the dashboard and the people doing the work are telling different stories, these four questions help move the conversation from the metric to what produced it and what is still producing it.
What decision or priority set this in motion?
Trace the result back to the leadership decision that created the conditions producing the number.
01
What conditions did it create?
What was the environment the people doing the work were operating inside when this result was produced?
02
How did the people adapt to keep the work moving?
Every workaround is data. Every informal process records a gap between what was designed and what the work required.
03
What remains in place to produce the number again?
After everything the organization has already done to address the issue, what is still there?
04
"What leadership decisions created the conditions this data is now reporting?"

WHERE TRUTH DRIFT™ COMES FROM
Introduced in The Data Trap
I introduced Truth Drift™ in The Data Trap: Why Healthcare Systems Are Drowning in Data But Starving for Truth, published in 2026. The book explores what happens when information loses meaning as it moves through an organization and how that shapes the decisions leaders make.
Every week in The Data Trap newsletter, one story from inside a healthcare leadership room demonstrates the pattern in practice. Published every Tuesday on LinkedIn.
WHEN THE SAME PROBLEM KEEPS COMING BACK
The question worth asking is what is still producing it.
The Organizational Performance Diagnostic works backward from the performance result to identify the specific leadership decisions and conditions that produced it, before the next initiative is designed to address a problem leadership does not yet fully understand.
